Community Living Services

We assist people in small, individualized, relatively permanent living settings, Emphasis is on giving each individual only as much service as is necessary to maintain as independent a lifestyle as possible. Teaching and supports are directed toward achieving individual preferences in skills such as financial management, health and safety, personal growth, community living and communication. Environments can range from limited supervision/support to full-time assistance.

Supported Living

The individual lives in his or her own home or apartment or with roommates of the person’s choosing. Staff is scheduled most, if not all days of the week for periods of an hour up to several hours per day, as funding allows and as decided by the person and his or her team. Supported living does not typically include staff providing overnight supervision.

Assisted Community Living

The individual receives funding to enable 24 hour supervision/support when the individual is not in day services. The person lives in his or her own home or apartment or in an agency managed residence. The individual may have from 1-2 roommates. Generally the needs and supports are more intensive in Assisted Community Living.

Adult/Children’s Family Home

Individuals live with a family providing significant supervision and support, usually on a 24 hour basis. MNIS contracts with the home provider after he or she has been certified by State Health and Human Services. The environments may be designed for one or several persons with developmental disabilities.

Medical Services

We believe that all people have the right to optimal health care from a physician of their choice. The Individual Program Planning Team will determine the need for physician and specialist referral within state guidelines. The Medical Services Facilitator is responsible for assisting the individual in ensuring that health needs are met and required assessments and follow-up appointments are maintained.
